Four copies of the form are included in this booklet. It is intended for each member of the family to complete one and turn it in. Additional copies are available. Remember to include your name, phone number and e-mail address if you have one. If already serving in a capacity and you wish to continue, please note it on the form. If your talent is not listed, or your desire to volunteer in a different capacity not listed, please write it in on the form.
